Public Records Request Form

Requests for public records may be made in writing, by phone, e-mail, fax, or orally to the Public Disclosure Coordinator at:

Public Disclosure Coordinator
Washington State Department of Personnel
521 Capitol Way South
PO Box 47500
Olympia, Washington 98504

Email: publicrecords@dop.wa.gov
Phone: 360-664-6270
Fax: 360-586-4694

The request should contain the following information:
  1. The name, address, and telephone number or e-mail address of the person requesting the record;
  2. The date on which the request is being made;
  3. A description of the record(s) requested; and
  4. If the request is for a list of individuals, the requester must certify that the information will not be used for commercial purposes except as permitted by RCW 42.56.070(9).

A form is available for use in requesting public records but is not required. Click here to download a copy of the form:  Public records request form (Word file).  Once filled out, the form can be submitted by email, fax, or mail as described above.

How much will it cost?
For paper copies there is no charge for the first 25 pages, but it is fifteen cents ($.15) per page after that (two-sided copies count as two pages).  Click here for our rules on fees related to public records: WAC 357-07-050.

How long will it take?
It depends on the specific request.  Within five (5) business days of receiving the request, the Department will respond and either:
  • Make the requested record(s) available;
  • Make a portion of the records available in installments as they are assembled;
  • Deny the request; or

  • Acknowledge the request and provide a reasonable estimate of time needed to respond.
The Department may also ask for clarification if the request is unclear.
