Referrals to the EAP

Formal Referrals are when you as management refer an employee to the EAP for work performance problems. Notify the EAP by telephone or in writing that you have referred your employee so we can better understand what has been happening at work. When we have the referral information from you we can let you know your employee has either contacted us or not contacted us. Informal Referrals are when management suggests or refers an employee to the EAP, but the referral is not for work performance problems. With informal referrals, you will not be notified of EAP contact without your employee's consent. Self Referrals are when your employee independently contacts the EAP for a confidential appointment. Job performance problems may or may not exist. You will not be notified of EAP contact without your employee's consent.
